- Ensure patients understand their diagnoses, medications, and treatment plans.
- Offer support and guidance for family members involved in patient care.
- Communication and Collaboration:
- Maintain clear and open communication with patients, families, and other members of the healthcare team.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
- Report and document patient progress and any changes in condition.
- Emergency Response:
- Be prepared to handle medical emergencies and provide immediate care.
- Collaborate with emergency services when necessary.
- Documentation:
- Maintain accurate and up-to-date patient records.
- Document all nursing interventions, changes in condition, and communication with other healthcare providers.
- Compliance:
- Adhere to all relevant healthcare regulations, standards, and ethical practices.
- Follow infection control procedures to ensure a safe and sanitary environment.
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid nursing license in the state of practice.
- Proven experience in home healthcare or a related field.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and adapt to varying home environments.
