Home Care Nurse

We are seeking a compassionate and skilled Home Care Nurse to join our healthcare team. Responsibilities:

• Medical Care:
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and other health indicators.
• Provide wound care, injections, and other nursing procedures.
• Patient Education:
• Educate patients and their families on proper care techniques and self-management strategies.
• Ensure patients understand their diagnoses, medications, and treatment plans.
• Offer support and guidance for family members involved in patient care.
• Communication and Collaboration:
• Maintain clear and open communication with patients, families, and other members of the healthcare team.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Report and document patient progress and any changes in condition.
• Emergency Response:
• Be prepared to handle medical emergencies and provide immediate care.
• Collaborate with emergency services when necessary.
• Documentation:
• Maintain accurate and up-to-date patient records.
• Document all nursing interventions, changes in condition, and communication with other healthcare providers.
• Compliance:
• Adhere to all relevant healthcare regulations, standards, and ethical practices.
• Follow infection control procedures to ensure a safe and sanitary environment.

Qualifications:

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid nursing license in the state of practice.
• Proven experience in home healthcare or a related field.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and adapt to varying home environments.
