Position / Designation: Registered Nurse
We are seeking a compassionate and dedicated Registered Nurse with a valid DHA license to join our team. License: DHA License / Eligibility letter as a Registered Nurse
Certificate: Attested Bachelor’s degree in Nursing or equivalent.
Experience: Minimum of 2 years of experience as a Registered Nurse, preferably in a home health care setting.
Skills:
- Excellent clinical skills and knowledge of best practices in home health care.
- Strong interpersonal and communication skills, with the ability to build rapport with patients and their families.
- Compassionate, empathetic, and patient-centered approach.
- Ability to work independently and make sound decisions in a home care environment.
- Proficient in using electronic medical records (EMR) systems.
Responsibilities:
- Provide compassionate and comprehensive nursing care to patients in their homes, following established care plans and protocols.
- Conduct thorough assessments of patients, including physical examinations, vital signs monitoring, and medication management.
- Administer medications, treatments, and therapies as prescribed by physicians and ensure accurate documentation of all interventions.
- Collaborate closely with the interdisciplinary team to develop and implement individualized care plans, setting appropriate goals for patient care.
- Educate patients and their families on disease management, medication administration, and necessary lifestyle modifications.
- Monitor and evaluate patient responses to treatments, adjust care plans accordingly, and communicate any changes or concerns to the healthcare team.
- Maintain accurate and up-to-date patient records and documentation of all care provided.
- Effectively communicate and collaborate with physicians, therapists, and other healthcare professionals involved in patient care.
